To confirm your booking, please complete the Client Intake and Consent forms for reservation to be valid. No appointments are confirmed until these forms are submitted. All cancelled appointments are charged a $50 cancellation fee.

If you would like assistance with creating your booking please call or stop by the office and we will schedule your appointment.
